Corrective Action Kit: NCR, 5 Whys and CAPA Log

A root cause analysis workbook with 5 Whys and fishbone worksheets, plus a nonconformity report form, CAPA log and effectiveness verification record.

A nonconformity comes back when the instance is fixed and the cause is left in place. This kit takes you from a clearly written nonconformity, through a root cause you can defend, to a corrective action that has been checked and shown to work. It fits clause 10.2 of ISO 9001, ISO 14001 and ISO 45001, and any other management system that expects nonconformities to be corrected, analysed and prevented from recurring.

What is inside

Root Cause Analysis Workbook — guidance on the difference between correction and corrective action, a problem statement form, a worked 5 Whys example, a blank 5 Whys worksheet, a fishbone worksheet across six cause categories (people, method, machine, material, measurement, environment), a 10-point check on whether your root cause is good enough, and a cause-to-action plan.

NCR Form and CAPA Log — four templates: a nonconformity report form, a CAPA log, an effectiveness verification record and a CAPA summary for management review.

Both documents as a designed, print-ready PDF and as an editable Word file.

Who it is for

Quality, HSE and compliance managers, process owners who have been handed a nonconformity to close, internal auditors who review corrective actions, and consultants who want one consistent set of forms for their clients.

How to use it

Raise the nonconformity on the NCR form: state the requirement, the evidence and why it does not conform. Deal with the instance straight away — that is the correction. Then take the workbook to the people who do the work, write the problem statement, and ask why until you reach something the organisation controls and can change. Test the result against the 10-point check before you agree the action. Record the action in the CAPA log with one owner and one date, and set a later date to verify that it worked. Close the nonconformity only after that check.

The kit does not replace your own procedure or the requirements of the standards you work to. Check which edition of each standard applies to you.

Questions people ask

Is the Corrective Action Kit: NCR, 5 Whys and CAPA Log really free?

Yes. There is no payment and no trial. Tell us your name, work email and company once, and every document in the Free Zone is yours to download.

What format are the documents in?

This kit has 4 documents in PDF and DOCX format. The Word, Excel or PowerPoint files are fully editable, so you can adapt them to your organisation.

Can I use it inside my company?

Yes. The licence covers one organisation with unlimited internal use — print it, share it with colleagues and adapt it. Please do not resell it or republish it as your own.

Does this make us compliant with ISO 9001?

No document can do that on its own. It is a practical working tool to help you prepare, check and improve. Conformity with ISO 9001 is decided by an audit of your actual system, and the standard itself remains the authoritative text.
