How to use it
Decide first which suppliers matter: the extent of control should reflect how much a supplier can affect your product or service. Send the questionnaire to the important ones, or complete it yourself during a visit, and ask for evidence behind the answers. Score the result, record the decision and enter approved suppliers on the list. Then monitor real performance — quality, delivery, service — and re-evaluate at the interval you have set. When a supplier delivers a nonconformance, raise a corrective action request and check that the fix worked.
The weights, score bands and example entries are there for you to adapt. ISO 9001 has been under revision — check whether a newer edition has been published and what transition arrangements apply to you.
